Engages and assists young adults as they move into adulthood. Traditional services provided include diagnostic assessment, counseling, case management, supported employment, as well as non-traditional activities such as outreach and engagement, service coordination, and orientation.

Lake Health provides a Primary Care Clinic right here at Crossroads Health. The medical clinic provides services for the management of chronic diseases, treatment of common illnesses and conditions, preventative care and wellness screenings.

A temporary break for parents caring for children with emotional or behavioral disorders.
Adults
The Supported Employment program serves adults within Lake County looking to gain or maintain employment. We work with each individual to determine their interests and assist in identifying barriers to better prepare for the employment process.
The Supported Employment Program looks to develop relationships with employers throughout the community to provide both employers and individuals within the program an ideal match. Whether looking into educational opportunities, preparing for work and the job search process or needing assistance maintaining work while overcoming barriers, the Supported Employment program will meet with each individual at their level of readiness.

Temporary foster care placement to provide safety, structure, and treatment until a family can be reunited.

Gives education, guidance, and consultation for families and community members to manage child development and mental health issues.

Specialized treatment to help families with children impacted by early trauma, who are often in kinship, foster or adoptive placements.

Treatment for children who have experienced sexual trauma or who exhibit concerning sexual behaviors.
