What to bring to your first appointment.
1. A copy of your ID and insurance card.
2. A list of all your current medications.
3. Guardianship paperwork if applicable.
4. Previous treatment records such as ADHD testing, recent lab work or hospital discharge paperwork if applicable.
What to expect in your first appointment.
We begin by conducting a psychiatric evaluation to learn more about your health needs, these appointments are scheduled for an hour. At the end of that appointment, you will receive an initial assessment and recommendations for next steps including prescription medications, ordering labs and referrals to additional services. Your psychiatry provider will discuss options to support you making an informed decision about your healthcare.
Services provided by Psychiatry.
1. Assessment and evaluation of physical and behavioral health symptoms and needs.
2. Management of medications including education on expected results and side effects.
3. Ongoing monitoring of medications, side effects and other health needs.
4. Post hospitalization follow up and supportive services.
5. Referrals to additional behavioral health services such as counseling or case management, Primary Care or other specialty medical services.
6. Onsite laboratory services for lab draws as well as point of care testing.
7. Nursing care to support behavioral and physical health care needs including nutrition education, medication education, medication monitoring and administration of medications.
Scheduling with your psychiatry provider.
The Crossroads Health Psychiatry Team believes that to be helpful we need to be accessible to our patients; that is why we use Express Scheduling. This is an evidence-based scheduling model that ensures patients can get access to follow up appointments within two weeks or less.
How Express Scheduling Works:
· At the end of your appointment with your prescriber, they will give you a date to call to schedule your next appointment.
· If you are signed up for text message reminders you will receive a text message shortly after your appointment with this call back date and you will receive another text message the day you should call to schedule.
· When you call in, an appointment will be scheduled within the next 2 weeks, ensuring you can be seen within the recommended time.
· If you have an urgent need to see your provider or an earlier return appointment is needed, you can call and be scheduled within the 2-week scheduling window.
· We also offer nursing walk in appointments for urgent or emergent needs Monday – Friday 9:00 – 4:30.

I missed a follow up appointment, now what?
Sometimes patients miss their follow up appointments or find that they are cancelling the day of. We encourage patients to avoid this as much as possible but if it does happen, we offer daily No-Show Clinics with our highly qualified nurses. Gone are the days of needing to wait to get a new appointment we can see you any day Monday – Friday providing the security to our patients to know that they will have consistent access to their medications and comfort that their health needs or presence of side effects are being monitored and responded to.
At the end of these brief appointments, you can reschedule with your provider, and our nursing team can help you get a bridge script of your medication until your appointment. In these appointments, we can also help address any barriers to consistently attending appointments, as your access to care is our priority.
No Show Clinic Schedule
We welcome you to walk in Monday – Friday 9:00 – 4:30 but we do offer dedicated times for the No Show Clinics as well.
