EUCLID, Ohio —The City of Euclid is excited to announce the opening of a new Federally Qualified Health Center (FQHC) access point through a partnership between Crossroads Health and Moore Counseling & Mediation Services, Inc. (MCMS). This new facility, scheduled to open in January 2027 at 22639 Euclid Avenue, will bring comprehensive healthcare and support services closer to Euclid residents.
The new health center will serve as a seamless entry point for individuals and families seeking a wide range of services, including healthcare, behavioral health treatment, recovery support, and other essential services. By integrating multiple services under one roof, the partnership aims to reduce barriers and enhance access to holistic care.
The new facility will provide the following services:
- Primary care services
- Behavioral health counseling for children, adolescents, and adults
- Psychiatry and medication management
- Substance use disorder screening, early intervention, treatment, and recovery support
- Care coordination and case management
- Presumptive Medicaid eligibility and enrollment assistance
- Employment and vocational services
- Community resource navigation and referral services
Crossroads Health, which became a Federally Qualified Health Center in 2026, is committed to delivering affordable and comprehensive healthcare, regardless of an individual’s ability to pay. The center will offer a sliding fee discount program to eligible patients, aiming to alleviate financial barriers to care.

In addition to direct services, Crossroads Health and Moore Counseling & Mediation Services, Inc. will establish a Community Advisory Committee comprising patients, community members, healthcare professionals, and local stakeholders, to provide ongoing feedback and ensure that the services remain responsive to community needs.
