Crossroads Health celebrates 5 years of Primary Care services for individuals and families of all ages at its Integrated Healthcare Center, located at 9220 Mentor Avenue in Mentor. Started in 2021 by visionaries Dean Visk, MSN, RN and Kelley Pastor, PA-C and have led the team to the successful healthcare center that it is now.
Same-day and next-day appointments are often available, making it easier for individuals and families to get the care they need, when they need it. Patients can receive annual wellness exams, treatment for common illnesses and injuries, preventive care, and prescription services, all in one convenient location.
“Our goal is to make healthcare accessible, convenient, and connected,” said Kelley Pastor, PA-C. “We care for the whole person, helping patients address both their physical and emotional well-being.”
The Primary Care team includes a Physician Assistant, Licensed Practical Nurse (LPN), and Medical Assistant, who work together to provide high-quality care in a welcoming environment.
Crossroads Health offers flexible scheduling throughout the day and reserves appointment availability for walk-in patients whenever possible.
