Care for Children, Adults, and Families
Crossroads Health offers many types of care and support for children, adults, and families.
Our programs include mental healthcare, addiction treatment, primary care, early childhood programs, and residential care. We also connect people with resources and services in the community when they need support beyond what we provide.
You may be looking for one type of help or several types. Our teams work together to help you find the care that fits your goals and stage of life.
Early Childhood and Youth School-Based Services
Children need the right support at the right time. Our early childhood and school-based programs help children build skills, manage challenges, and feel more successful at home, at school, and in their communities.
Services are designed for children and families who may be experiencing emotional, behavioral, or developmental challenges. Care focuses on each child’s strengths and goals.
Our early childhood and school-based services include:
- Mental health support for young children and youth
- Therapeutic Day Treatment and Academics for ages 5-18
- Infant and Early Childhood Mental Health Consultation for parents, childcare centers, and preschools
- Family involvement, support and workshops for parents and guardians
- Help Me Grow early intervention services
- Home visiting for pregnant women and families with children (birth to age five)
- Coordination with families, schools and other local providers
- School-based Behavioral Health in partnership with school districts
We know children do not experience challenges in isolation. Family, school and community all play a role in a child’s well-being. When appropriate, we work with parents, caregivers, schools and other providers to help create consistent support.
Integrated Health Services
Your mental health and physical health can affect each other, so good treatment should keep both in mind. At the same time, getting the right care should not require you to figure everything out on your own.
That’s why Crossroads Health starts with an open-ended intake session, where we listen to each individual person and determine what services can help. From there, we’ll help you manage your healthcare as a whole person. If you require something outside of our services, our team will connect you with the right programs in the community.
Our health services include:
- Primary care
- Mental healthcare
- Addiction treatment
- Pharmacy services
- Medication management
- Health screenings
- Care coordination
- Referrals to community providers
When you receive more than one type of care at Crossroads Health, our teams communicate and work together when appropriate. This approach makes it easier to understand your care and take the right next step.
Residential Services
Sometimes people need more support than outpatient care can provide. Crossroads Health offers residential services for individuals who need a safe, structured place to focus on treatment and recovery.
Residential programs provide support around the clock. Daily programming can include clinical services, counseling, recovery support, education, skill building and time for healthy routines.
Care may include:
- Individual and group counseling
- Family support
- Recovery education
- Mental health support
- Medication support
- Life skills
- Connections to community resources
Residential care is not meant to be a one-size-fits-all experience. Staff work with each person to understand their goals and help prepare them for the next step in recovery. Family involvement may also be part of residential care when appropriate. Support from family and other trusted people can help strengthen recovery and make the transition home easier.
Find the Right Care
People often require more than one kind of support.
Someone may need therapy and primary care. A parent may seek help finding services for a child while also finding support for themselves. A person in addiction treatment may look for help with mental health, medications, housing, or other issues.
Crossroads Health works to make those connections easier.
Our providers and programs work together when possible. We also build relationships with schools, healthcare providers, community organizations and other partners.
If there is a service we do not provide, we can help you find the right resource.
